Procedure for Reporting RIDDOR Reportable Incidents

 

What to report

Generally speaking, only those installations where work has recently been carried out, and as a result of that work pose an immediate threat to gas users, should be reported.The sorts of situations that should be reported are:

  •  

Instances where the use of unsatisfactory fittings or poor workmanship result in a gas escape outside the tolerance of a soundness test;
  •  
Uncapped, open ended pipes connected to the gas supply;
Appliances that are spilling products of combustion, or show past signs of having done so with no evidence that the cause has been rectified;
  •  
Defective flues or chimneys that are not clearing flue gases;
  •  
Appliances that should be flued, but are not.
  •  
Appliances that show signs of combustion problems because of inadequate permanent ventilation;
  •  
Appliances that are not suitable for use with the gas supplied (e.g. natural gas appliances being used with LPG);
  •  
Appliances that have had a safety device, such as a flame-failure device, made inoperative;
Appliances that are connected to the gas supply by a connection made of unsatisfactory material, such as garden hose;
  •  
Appliances that are dangerous through faulty servicing.

In addition, the HSE have requested that the following unsafe situations should be reported under RIDDOR.

All illegally installed conventionally flued appliances in bathrooms and shower rooms (those known to have been installed after 24th November 1984).

Appliances in rented accommodation that have not been safely maintained.

When to report

By law (RIDDOR), we must send a report of any dangerous fittings within 14 days of finding them.

How to report

Reports should be sent directly to the HSE on an F2508G form.

Forms are available from the area office. The form is to be copied at the office. One copy is to be retained with the job recording form and is to be forwarded to the Supervisor. The second copy is to be sent to the local HSE office in relation to the incident location e.g.:
